You acknowledge that you have received the separate documents entitled “Point Pick-Up Technologies, Inc. Background Check Disclosure” and “Summary of Rights pursuant to the Fair Credit Reporting Act” and certify that you have read and understand both of these documents.

You hereby authorize Point Pick-Up and any credit reporting agency or investigative consumer reporting agency directed by the Company to obtain your “consumer report” as defined by the Fair Credit Reporting Act (“FCRA”), 15 U.S.C. § 1681a, or “investigative consumer report” as defined by the FCRA, 15 U.S.C. § 1681a, or other report on your background as permitted by applicable state law.

You further hereby authorize all previous employers, educational institutions, consumer reporting agencies, and other persons or entities having information about you to provide such information to the Company or such other entity that obtains information for the Company.

You understand that you may receive a free copy of any consumer report about you procured by the Company. The Company obtains consumer reports from Checkr, Inc., located at 2505 Mariposa Street, San Francisco, CA 94110 and may be reached at hello@checkr.com or at 844-2-CHECKR.

You understand that you have the right, upon written request to the Company made within a reasonable period of time (not to exceed 30 days) after agreeing to this authorization to receive a written disclosure of the nature and scope of any investigation.

You may view the file maintained on you by the consumer reporting agency referenced above during normal business hours. You may also obtain a copy of this file upon submitting proper identification and paying the costs of duplication services, by appearing at the consumer reporting agency in person, by mail, or by telephone. The agency is required to have personnel available to explain your file to you and the agency must explain to you any coded information appearing in your file. If you appear in person, a person of your choice may accompany you, provided this person furnishes proper identification.
